B.E.S.T. in Class Eligibility

Telligen strives to instill sustainable quality and performance improvement practices for our Telligen QI Connect™ participants. In 2023, the B.E.S.T. in Class recognition award was introduced. It raised the bar and challenged participants to sustain robust infection prevention practices and training, complete a gap analysis for their emergency preparedness plans, receive the Blue Ribbon in COVID-19 Vigilance award for three quarters and address transitions in care by meeting the criteria for preventable emergency department visit.  In April 2024 criteria for Blue Ribbon was changed and transitions in care will now address 30-day all-cause rehospitalization rates.

Additional nursing homes will be added to the B.E.S.T. in Class recognition list in August 2024.

 

Transitions in Care:

Improvement in 30-day All-Cause Rehospitalization Rates

Performance is measured using MDS data.

    • Denominator: Entry assessments data codes; A1600 Entry date = completed field, A0310F value of (01) for residents who entered the facility FROM a hospital during the 3-month reporting time period, A1805 entered from hospital = (04,07,11)
    • Numerator: Discharge assessments = (A0310F value of 10 or 11), A2000 discharge date = completed field, A2105 discharge status = (04, 07, or 11) and only include unplanned discharges (A0310G=2)

MDS rolling 3-month periods for B.E.S.T. in Class:

    • Baseline: October 2023 – December 2023, plus 30 days for readmission (through January 30, 2023)
    • Post-Baseline 1: November 2023 – January 2024, plus 30 days for readmission (through March 1, 2024)
    • Post-Baseline 2: December 2023 – February 2024, plus 30 days for readmission (through March 30, 2024)
    • Post-Baseline 3: January 2024 – March 2024, plus 30 days for readmission (through April 30, 2024) 
    • Final: February 2024 – April 2024, plus 30 days for readmission (through May 30, 2024)
